Message Area:This
area may contain billing messages. These messages relay important
information regarding your telephone service.
Monthly ChargesThis
area lists each recurring monthly charge posted to your account.
This section identifies each charge and shows the number
of units charged for, the cost per unit, and the total cost.
This area may be followed by a section for Other Charges
and Credits, which displays any unusual, non-recurring charges
and credits posted to your account during this billing cycles.
It includes a descriptions of each charge or credit and the
amount.
Toll Section: This
section shows all the toll calls you placed during the current
billing cycle. In addition to details for each toll call,
it identifies calls that qualified for discount plans by
marking them with at special symbol which varies depending
on the plan and carrier. It also shows the total amount
of any discount. You may have separate sections or pages
of toll calls for calls handled by different long distance
carriers. Subtotals are provided for each category or toll
calls.
Identification Information The information in this section appears on every page of your bill. In addition to your name, number, billing date, and payment due date, it includes the name and address of our telephone company and a number you can call for inquiries.
Payment History/Bill Summary:The
Payment History section contains charges from your previous
bill and any payments received against those charges since
the last billing date. The Current Billing Summary lists
your current charges by type and the total amount due.
Carrier Selection:This
area is reserved for display of information on your chosen
interstate long distance carrier and local long distance
carrier. This feature may appear on your bill beginning in
late 1999.
Return Stub: Detach
this section at the perforation and return it to our office
with your payment. This stub displays information we need
to post the payments to your account properly. This includes
your name and address, telephone number, and the amount you
owe. There is also space for you to write in the actual amount
of your payment 9unless the bill will be paid automatically
by your bank). Our address also appears on this page so that
it will show through the window in your return envelope.
